How to Submit Your Paperwork to the CD Program

The procedure for submitting the paperwork to the CD program is:

• The CAP Form 83 must be computer-generated, printed on one sheet of paper (front and back). Original signature on the form is required.

• If any of the questions in #19 and/or #20 are answered Yes, a letter of explanation must accompany the Form 83 with all details. The applicant must sign and date this letter of explanation.

• Your Unit CD Coordinator (or Unit Commander if the CDC is unavailable) fills the CD Check List. 

• Your Unit CD Coordinator (or Unit Commander if the CDC is unavailable) checks for accuracy and correctness before mailing it to the Area/Group CD Coordinator for processing.

• The Area/Group CD Coordinator checks the paperwork, then mails it to Lt. Col. Earl Sleeper, TXWG CDO.

• Never mail any of this paperwork directly to TXWG Headquarters.
